With the increase in COVID cases in our community, we are trying to make sure that we maximise the face to face time that we can spend with you, while ensuring that we keep everyone safe. To enable us to do this, we have set up an online questionnaire that we will ask you to complete prior to each appointment. Once completed, a secure PDF will be created and will be available for your midwife or doctor prior to your consultation.
